Intake: Conduct intake assessment to understand the unique needs and circumstances of each family. This includes gathering relevant information, reviewing case history, and identifying key issues.
Referrals and Resource Coordination: Connecting families with necessary community resources and support services, such as counseling, parenting classes, housing assistance, healthcare, substance abuse treatment, etc. Coordinating with relevant agencies and organizations.
